GLP-1: Survodutide
Survodutide is a weekly weight-loss shot still in testing. In 3 big trials, people lost more weight than on placebo. It is not FDA approved.

Still in clinical trials. Not FDA approved yet.
3 big phase 3 trials with about 1,700 people, all vs placebo. The FDA has not reviewed them yet.
About 1,700 people for up to 76 weeks. Not yet FDA reviewed.
These are 3 different questions. FDA status is about the law. Proof is about whether it works. Safety data is about how much we know about its risks. A lack of safety data does not mean it does not work, and "not approved" does not mean "illegal."

What is it?
Survodutide is a lab-made peptide medicine from Boehringer Ingelheim and Zealand Pharma. It acts on two body signals: GLP-1 and glucagon.
Why do people use it?
People are interested because big final-stage (phase 3) tests showed weight loss.
Does it really work?
There is good early evidence from 3 big final-stage (phase 3) trials. In adults with obesity, people on the higher dose lost about 13% of their weight in 76 weeks, vs about 5% on a fake shot (placebo). In adults with obesity and type 2 diabetes, they lost about 10%, vs about 4%. As of October 22, 2026, it is not approved, and we found no public news that the company has asked the FDA to approve it for weight loss.
Studies in people
| Study | People | What they found | Limits |
|---|---|---|---|
| Randomized trial (SYNCHRONIZE-1), adults with obesity | 725 | Lost 12.2% to 13.0% vs 5.4% on placebo at 76 weeks. | Stomach side effects in 81% to 90% vs 48%. Paid for by the maker. |
| Randomized trial (SYNCHRONIZE-2), adults with obesity and type 2 diabetes | 752 | Lost 8.2% to 9.8% vs 3.9% on placebo at 76 weeks. Blood sugar (A1C) dropped more. | Stomach side effects in 73% to 78% vs 39%. Paid for by the maker. |
| Randomized trial (SYNCHRONIZE-MASLD), adults with obesity and fatty liver | 216 | Lost 8.7% vs 1.4% on placebo at 48 weeks. Liver fat went down more. | Shorter trial. Liver results are not proof it prevents liver disease. Paid for by the maker. |

Is it safe? Side effects
- In the obesity trial, stomach problems happened in about 8 or 9 of every 10 people on survodutide, and in about 5 of 10 on the fake shot. They were usually mild to medium.
- In the trial of people with type 2 diabetes, stomach problems happened in about 7 or 8 of every 10 people on survodutide, vs about 4 of 10 on placebo.
- No deaths were reported in the obesity trial.
- The FDA warns that unapproved GLP-1 type drugs sold online may be fake or unsafe.
Is it legal in the US?
Survodutide is not FDA-approved. (The FDA is the US agency that checks if medicines are safe and work.) Right now you can only get it legally in an approved clinical trial.
What "not approved" does and does not mean
- "Not FDA approved" is not the same thing as "illegal." It means the FDA has not reviewed proof that it works and is safe.
- Outside the narrow pharmacy rules below, selling it as a medicine for people breaks federal law. The FDA has warned companies that sold unapproved drugs labeled "for research" or "not for human consumption" to people. (FDA)
- A regular pharmacy may only mix (compound) a drug from a raw ingredient that has an official quality standard, is part of an FDA-approved drug, or is on the FDA’s 503A list. Big "outsourcing" pharmacies (503B) have their own list. (FDA)
- The FDA says that in most cases it is illegal for people to bring unapproved drugs into the US for their own use. (FDA)
- State rules differ. State boards of pharmacy oversee most pharmacies day to day. Sports leagues and the military have their own bans. (FDA)
Banned in sports? Drug tests
Likely banned in sports.
Survodutide is not named on WADA's list. But it is still a test drug and not approved anywhere. WADA's S0 rule bans drugs "under clinical development" with no approval, at all times. From Jan 1, 2027, that rule also names "peptides."

More questions
Is survodutide the same as retatrutide?
No. Survodutide acts on glucagon and GLP-1. Retatrutide acts on those two plus GIP.
When could survodutide be approved?
No date is known. The maker said in April 2026 that the FDA gave it Fast Track and Breakthrough Therapy status for a liver disease called MASH. Those speed up review. They are not approval.
Is survodutide a GLP-1?
Partly. It acts on GLP-1 and glucagon.
