Which tirzepatide was recalled in October 2026?

  • Apothecary Pharma LLC (Cary, North Carolina) recalled 22 vials of tirzepatide injection, 18 mg/mL, 4 mL multi-dose vials.
  • Lots AP-202622 to AP-202644. Lot AP-202626 is not on the FDA list. Beyond-use dates run from October 7 to November 26, 2026.
  • Reason: "lack of assurance of sterility." The recall started September 24, 2026. FDA classified it as Class II on October 8, 2026, and listed it in its October 14 Enforcement Report.
  • The same recall lists 1 vial of NAD+ injection (lot AP-202647). NAD+ is not a peptide.
  • FDA also lists a Class II recall from Carolina Infusion LLC (Ridgeland, South Carolina) of pharmacy-made tirzepatide, semaglutide, and sermorelin syringes, classified the same day.

What is a Class 2 recall?

Recall classWhat FDA says it means
Class IThe most serious. FDA says there is a reasonable chance the product will cause serious health problems or death.
Class IIFDA says the product may cause short-term or medically reversible health problems, or the chance of serious harm is remote.
Class IIIFDA says the product is not likely to cause health problems.

The company runs the recall. FDA classifies it and posts it in its weekly Enforcement Report.

Are compounded GLP-1s safe?

Compounded GLP-1 drugs are not FDA approved. The FDA does not check them for safety, effectiveness, or quality before they are sold. That is a separate question from whether one batch was recalled.

Most recalls in our list are pharmacy-made shots. Most were for sterility problems, particles in the liquid, or endotoxins (bits of bacteria that can cause fever).

FAQ

Is Zepbound or Mounjaro being recalled?

We found no 2026 recall of Zepbound or Mounjaro in FDA’s Enforcement Report data (last updated October 14, 2026). The tirzepatide recalls we found were pharmacy-made versions. Brand-name Wegovy had a small 2-lot recall in January 2026, listed in the tool.

What does "lack of assurance of sterility" mean?

It means the maker could not show that the product was made in a way that keeps germs out. It does not always mean germs were found. Shots must be sterile, so it still matters.

My lot is not on the list. Does that mean it is safe?

No. It only means it does not match the recalls in our list. New recalls can be posted at any time, and our list covers only GLP-1 drugs and peptides. Check the FDA links on this page, too.

Where is the lot number?

Look on the vial or box label for "Lot." Pharmacy-made drugs often show a "BUD" (beyond-use date) instead of an expiration date. Lot numbers can mix letters, numbers, dashes, and @ signs, like AP-202630 or 07212026@18. Type it exactly as printed.

Is NAD+ a peptide?

No. NAD+ is made of 2 nucleotides, not amino acids. We include it only because it is in the same Apothecary Pharma recall. Glutathione is different: it is a tiny chain of 3 amino acids (a tripeptide).

Does a recall mean the pharmacy is closed?

No. A recall covers the products and lots listed. It does not say anything about other products by itself.

Is my search saved or sent?

No. The checker runs in your browser. Nothing you type is saved or sent to us.
